How to Find a Network Provider

3. Under Continue as a Guest, enter your home information (zip, city, county, or state) and desired search range and click Search.
4. Select your plan from the list provided and click Continue.

1. Click on the Locate a provider or Create a directory button on the right side of the page.
2. Enter your Network Type and press Start Now.
3. Choose your Provider Type and select a zip code or state. Click Search Now.

PHCS / PHCS Out of Area

1. Click on Select Network on the left side of the page and answer the populated questions.
2. Enter your search criteria (name, specialty etc.) and enter your zip. Click the magnifying glass to search.

PHCS – Illinois

1. Choose your Provider Type and select a Provider City and Provider State.
2. Click Search.

Sagamore Health Network – Plus

1. Search by provider name or facility name, or search by location, zip code, and specialty.
2. Click Search.

1. Click on Search SuperMed PPO Access.
2. Enter the provider information and click Search Providers.
